SNMHI Coach Medical Home

Coach Medical Home is a web-based curriculum released in 2013 and designed to equip practice facilitators with public domain tools and knowledge to effectively support practices that are in pursuit of medical home principles and recognition.

The curriculum was created by the MacColl Center in conjunction with Qualis Health, and is based on the Safety Net Medical Home Initiative (SNMHI) Framework for Practice Transformation.  The SNMHI framework consists of eight change concepts (below) to guide primary care transformation in a variety of settings.

These materials are designed both for facilitators working in primary care associations, health center controlled networks, or other coalitions, and for facilitators working within a health center to improve care.  Though many of the tools and resources are broadly useful, challenges specific to safety net practices are highlighted throughout the curriculum.
