Privacy Statement
The Improving Chronic Illness Care Web site was designed to help delivery systems improve their care for individuals with chronic conditions through the dissemination of information to clinicians, researchers, and others interested in healthcare quality improvement.

We collect no personally identifiable information about you when you visit our Web site unless you provide us with this information. We do not share personal information with third parties except as would be required in the event of a legal process.

Information Automatically Collected and Stored
We collect standard server log information (i.e., domain name or IP address from which you access the Internet, the date and time you access our site, the pages you viewed). We use anonymous browser “cookies” to track file usage from all visitors.

All automatically collected information is collected on an aggregate basis only. Our Web site logs are not personally identifiable and we make no attempt to link them with the individuals who browse our site. For example, we track which pages are most popular among visitors as a whole, but pages viewed are not identified with individual users. With email inquiries, we store the inquiry and the email address information so that we can respond electronically.

Email Registration
Your name and email address will not be released, sold, loaned, or otherwise distributed to any person or organization outside ICIC. You may contact us any time if you’d like to have your name and email address deleted from our records.

In addition, advertisers and sponsors may not suggest that either Improving Chronic Illness Care or improvingchroniccare.org endorses a product, service or company.

If we change our privacy policy, we will post those changes to this privacy statement and any other place we deem appropriate so our users are always aware of what information we collect and how we use it, and under what circumstances (if any) we may disclose it.
