Thank you for visiting Improving Chronic Illness Care!

Our funding for updating this website ended in 2011, but the resources we developed are still helping clinical practices from around the world to transform the care they provide to better serve patients with chronic illness. Past Programs

This archive of grants and meetings awarded or convened by ICIC serves as a reference for accessing information and materials from past events.

Grants Programs

The Collaborative Sponsorship Grants Program was designed to help organizations interested in sponsoring regional or local collaboratives.  Our Targeted Research Grants Program funded  peer-reviewed, applied research projects.

National Meetings

One of our objectives has been to build and support the field of chronic illness care:  integrating clinical, behavioral and organizational research into practice.  To that end, we designed a series of meetings to stimulate interaction among front-line practitioners, researchers, and policy-makers.  In addition to providing a venue for innovators across the country to connect with one another, goals for the "Innovations in Research and Practice Congress" program were to identify the most promising ideas for improving care and priority directions for research and policy development.

Co-sponsored by The Robert Wood Johnson Foundation and The Agency for Healthcare Research and Quality, our invitation-only meetings took place from 2002 to 2004.
