Patient-Centered Medical Home Assessment

The MacColl Center, in partnership with Qualis Health, led the Safety Net Medical Home Initiative (SNMHI) with the goal of transforming safety net clinics into Patient-Centered Medical Homes.  Early in the study, we identified the need for an assessment tool that could be completed by providers and other members of the care team to measure their progress in the eight change concept areas established as key components of a Patient-Centered Medical Home.

The instrument was modelled on the Assessment of Chronic Illness Care (ACIC), utilized by hundreds of teams to guide their Chronic Care Model-based improvement efforts.  Similar to the ACIC, each question in this instrument has four response levels, describing where a team might find itself on the journey to becoming a Patient-Centered Medical Home.

The PCMH-A was created to help clinical practices gauge their progress in implementation of the Patient-Centered Medical Home change concepts.  It is an interactive PDF that can be downloaded, completed, saved and shared.  This tool may be further revised and changed. and ICIC will post news of any updated versions.
